What are some terminal illnesses that do not respond well to pain relief?

Depressing question, but I’m trying to do background research for a story, and my online searching hasn’t really helped. The minor character in question would be elderly, have a terminal illness and unable to gain much relief from pain management. Any suggestions or directions towards helpful information would be really useful.

I need something a little more detailed than just cancer, please. Perhaps some description as to a particular type which really doesn’t respond well to pain management and why.
